Although Medicare pays for the full range of colorectal cancer screening methods, only 1 in 4 people over 70 with Medicare are fully screened according to recommended guidelines.
In a study sponsored by the American Cancer Society. researchers reviewed Medicare claims for over 153,000 representative beneficiaries who were originally identified in 1998. Among the group were 18,000 people who were at higher risk for colorectal cancer.
Between 1991 and 1997, before screening was covered by Medicare, 29.2 percent of the group was fully screened, including 76.7 percent of the increased risk individuals and 22.9 percent of other patients.
During the follow-up period, 1998 through 2004 after Medicare began paying for screening, only 25.4 percent of the group were up-to-date with recommended colorectal cancer screening.
People who had been screened during the earlier period, were more likely to have follow-up screening between 1998 and 2004. One in three of those screened previously received a follow-up screening exam compared to only one in five of those who had never been screened.
Most often, colonoscopy was used for screening. 17.6 percent had colonoscopies, 2.6 percent received flexible sigmoidoscopy, and 4.1 percent used a combination of methods. Fewer than one percent were screened with a yearly fecal occult blood test, and almost none had barium enema.
Screening was more common in younger people, in men, in whites, and in areas of higher income and educational level.
Gregory Cooper, MD, and his team at Case Western Reserve University in Cleveland concluded,
In a population-based cohort of Medicare beneficiaries, despite insurance reimbursement, there is significant underuse of colorectal testing. Given the ability of screening tests to reduce cancer incidence and mortality, continued efforts to promote screening are clearly warranted.
