Dr. Jennifer Huberty (Director of Science, Calm) and researchers at Arizona State University and Mays Cancer Center at UT Health San Antonio are conducting a study to test a smartphone meditation app prototype among cancer patients and survivors. They are inviting you to determine if you are eligible for our study. This will involve answering questions such as your current health status. You have the right not to answer any question, and to stop answering the questions at any time. Your participation in the eligibility screening is completely voluntary. If you choose not to participate in the eligibility screening at any time, there will be no penalty. There is no direct benefit of answering these eligibility questions nor are there any foreseeable risks or discomforts from participation in the eligibility screening.   If you are eligible for the study you will receive an email indicating your eligibility status as well as a link to the informed consent document. If ineligible, they will discard the information provided for us in the eligibility screening and will contact you via e-mail to determine your desire to participate in future studies. Your confidentiality will be maintained as your contact information will be kept separately from your survey responses. If you have any questions concerning the eligibility screening, please contact the research team. If you have any questions about your rights as a subject/participant in this research, or if you feel you have been placed at risk, you can contact the Chair of the Human Subjects Institutional Review Board, through the ASU Office of Research Integrity and Assurance, at (480) 965-6788. By clicking next and completing the eligibility surveys, you are consenting to our use of your information.
