What are Care Sequences?
4R Care Sequences® are effective, proven guideline-based care delivery plans for providers and patients. Care Sequences® exist for patients at diagnosis and at transitions to different treatments. Care Sequence® plans are part of the 4R Oncology® model, a set of proven tools for patients and their care teams to support personalized care planning and delivery. 4R is right information and right care for the right patients at the right time.
These tools incorporate national guidelines for direct cancer care, supportive care, health maintenance, and social care into actionable plans.
What’s included in Colorectal Cancer Care Sequences® for Treatment?
- Pre-printed templates, 1 page, in color, on a tear pad with 25 sequences each.
- A package of 3 Care Sequences to use based on your patient’s treatment plan:
- Systemic therapy
- Chemo and Radiation
- mCRC therapy
- Can be given in person or by telehealth
- Available in English, Spanish and several other languages
